XERF is a new monopolar radiofrequency device developed by Cynosure Lutronic and used for noninvasive soft tissue coagulation and tightening. It has gained early clinical traction in South Korea as a next-generation RF option for lifting and tightening facial tissues without numbing, needles, or downtime.

How XERF delivers energy

XERF uses noninvasive monopolar radiofrequency to heat soft tissue at multiple depths, reaching the shallow, middle, and deep layers of the skin, including the SMAS (Superficial Musculoaponeurotic System), according to histological analyses. In in-vivo studies, internal skin temperatures of 50°C, 60°C, and 70°C were measured using ultrasound-guided fiber-optic imaging, and histological analysis confirmed collagen remodeling with increased thickening and tightening of collagen bundles at multiple tissue depths.

Structural skin tightening concept

XERF is framed to offer “Structural Skin Tightening,” emphasizing effects that extend from the superficial skin layers down to deeper supporting tissues. Histological analysis from multiple clinical studies cited by the manufacturer indicates that the XERF system delivers RF energy to the SMAS, a foundational tissue layer beneath the skin that plays a key role in lifting and tightening sagging skin and reducing the appearance of jowls.

Customization and treatment experience

XERF incorporates Accurate Impedance Feedback for real-time analysis of the skin and offers three depth settings with ten intensity levels to tailor treatment to each individual patient. Clinical and marketing materials highlight visible results with treatments described as comfortable, noninvasive, and designed without the need for numbing, needles, or downtime.

Jane Yoo, MD, MPP, is a Korean American board-certified dermatologist and fellowship-trained Mohs surgeon based in Midtown Manhattan.
