Overfilled or “Pillow Face”: Can It Be Fixed?
Medically reviewed by Jane Yoo, MD, MPP, dermatologist and Mohs surgeon. Last reviewed 11/04/2026.
The short answer: Usually, yes. If the filler is hyaluronic acid, it can be dissolved with an enzyme called hyaluronidase, either fully or partly. Other kinds of filler cannot be dissolved the same way, and other approaches may be needed. A careful consultation first, then a conservative plan, gives the best chance of a natural result.
What “pillow face” means
Pillow face is the puffy, rounded look that can result when too much filler is placed, or when it is placed in a way that does not suit the face’s structure. Common signs include:
- Cheeks that look swollen and puffy rather than lifted
- A flattened or overly full midface
- Lips that look unnaturally large or lumpy
- Puffiness under the eyes
- A face that looks “done” or has lost its expression
Why it happens
- Too much product in one session or over time
- Filler placed in areas that did not need volume
- Product that migrates or accumulates
- Filler that was not a good match for the area
- Weight changes after treatment, which can change how the filler looks. [Link to Post 4 on Ozempic face.]
Can it be fixed?
It depends on what was injected.

Hyaluronic acid filler |
Can be dissolved with hyaluronidase, fully or partially |

Unknown product |
Needs assessment. Ask for treatment records if you have them |

Non-hyaluronic filler |
Cannot be dissolved the same way. A dermatologist can discuss other options |

Swelling only, soon after treatment |
May settle with time. Do not dissolve right away unless there is a safety concern |
Timing matters
Fresh swelling after filler can look worse than the final result. It often settles over a couple of weeks. A good injector will usually wait before deciding. If something looks wrong, such as severe pain, blanching, or a lacy skin pattern, treat it as an emergency and call right away.
What dissolving involves
- A consultation to understand what was placed, where, and what bothers you.
- Small amounts of hyaluronidase placed in the areas that need it.
- A short wait to see the effect, and a follow-up to fine-tune.
Some swelling and bruising are common. Dissolving can sometimes be done in stages so that you keep the volume you want.
After dissolving
Once the swelling is gone, your dermatologist can discuss whether you would like to rebuild volume more conservatively, use other treatments for structure, or leave things as they are. Do not rush to re-inject.

Use the checklist in 12 questions to ask before getting filler, choose a conservative plan, and add treatment in stages.
Frequently asked questions
Does dissolving filler damage my own tissue? Hyaluronidase breaks down hyaluronic acid. Your own tissue contains natural hyaluronic acid as well, so a skilled injector uses small, targeted amounts.
Can all fillers be dissolved? No. Only hyaluronic acid fillers can be dissolved with hyaluronidase.
How quickly can filler be dissolved? Effects are often seen within days, but fine-tuning takes follow-up.
What if I do not know what I had? Bring any records you have. Your dermatologist can examine and advise.
